Título: Intervención psicológica temprana en pacientes hematológicos candidatos a trasplante de progenitores hematopoyéticos

Resumen

Introducción: un trasplante de progenitores hematopoyéticos consta de una serie de procedimientos secuenciales basados en protocolos específicos que comportan unas características especiales. Este tratamiento,  ya de por si difícil por si agresividad, conlleva una elevada afectación a nivel psicológico en las personas que han de realizarlo. Objetivo. Pacientes candidatos a trasplante de progenitores hematopoyéticos (TPH) y sus familiares más cercanos. Pacientes ingresados en la Unidad de Trasplante Josep Carreras del Instituto Catalán de Oncología de Badalona. Metodo:  La situación de trasplante comporta ciertos riesgos a lo largo del tiempo que el paciente tiene que afrontar. Estos riesgos suelen estar relacionados con el sufrimiento que genera la situación en su globalidad, pero especialmente en las consecuencias de los efectos secundarios, la situación de aislamiento, la sensación de alejamiento temporal de la familia y entorno habitual y por supuesto, el impacto por riesgo de muerte. En nuestro abordaje clínico incorporamos una visita pre-trasplante del paciente y su nucleo familiar principal, con varios objetivos: el primero realizar una valoración psicológica del paciente y cribar  los principales factores de riesgo a nivel psicosocial.  El segundo prevenir posibles trastornos psicopatológicos que pueden aparecer durante el proceso de hospitalización y período posterior, así como favorecer un buen proceso de adaptación. En tercer lugar activar los recursos necesarios que la família del pacient pueda precisar ante la situación. Para poder valorar estos aspectos utilizamos los siguientos instrumentos de evaluación: entrevista semiestructurada, escala analógica-visual para valorar ansiedad y también para valorar depresión. Resultados:  en nuestra experiéncia clínica, los resultados obtenidos nos indican lo beneficioso que puede ser el realizar un abordaje psicológico integrado, prévio al trasplante,  tanto del paciente como de su núcleo familiar más cercano, con el objetivo de detectar los riesgos y fortalezas/recursos del paciente y su família,  así como prevenir posibles trastornos psicopatológicos que puedan aparecer durante el tratamiento y el período de hospitalización. Conclusiones: la atención psicológica temprana integrada, dirigida al paciente hematológico candidato a trasplante es indispensable para un adecuado abordaje integral de una situación compleja como es el tratamiento mediante el trasplante de progenitores hematopoyéticos.

Abstract

Introduction: a hematopoietic stem cell transplant consists of a series of sequential procedures based on specific protocols that have special characteristics. This treatment, already difficult due to its aggressiveness, entails a high psychological level of affectation in the people who have to carry it out. Objective. Candidate patients for hematopoietic stem cell transplantation (HSCT) and their closest relatives. Patients admitted to the Josep Carreras Transplant Unit of the Catalan Institute of Oncology of Badalona. Method: The transplant situation involves certain risks over time that the patient has to face. These risks are usually related to the suffering generated by the situation as a whole, but especially in the consequences of secondary effects, the situation of isolation, the feeling of temporary estrangement from the family and usual environment and of course, the impact of risk of death. In our clinical approach we incorporate a pre-transplant visit of the patient and his main family nucleus, with several objectives: the first to carry out a psychological assessment of the patient and to screen the main risk factors at a psychosocial level. The second is to prevent possible psychopathological disorders that may appear during the hospitalization process and the subsequent period, as well as to promote a good adaptation process. Third, activate the necessary resources that the patient's family may need in the situation. In order to assess these aspects, we used the following assessment instruments: semi-structured interview, visual-analog scale to assess anxiety and also to assess depression. Results: in our clinical experience, the results obtained indicate how beneficial it can be to carry out an integrated psychological approach, prior to transplantation, for both the patient and his closest family nucleus, with the aim of detecting the risks and strengths / resources of the patient and his family, as well as to prevent possible psychopathological disorders that may appear during treatment and the period of hospitalization. Conclusions: early integrated psychological care, aimed at the hematological transplant candidate is essential for an adequate comprehensive approach to a complex situation such as treatment by transplantation of hematopoietic progenitors.
